Thyroid Ultrasound in Korean Health Checkups: A Guide for Foreigners

Thyroid ultrasound is a common optional examination in Korean private health checkups.

It is quick, painless, does not use radiation, and is very sensitive for detecting thyroid nodules.

However, that sensitivity also means thyroid ultrasound needs to be used carefully. Small thyroid nodules are extremely common, and most are benign. Detecting a nodule does not automatically mean that a biopsy or surgery is necessary.

It is also important to distinguish between a test that is commonly offered and one that is recommended for everyone.

Routine thyroid cancer screening with ultrasound is not recommended for all asymptomatic adults. The decision to undergo screening should therefore depend on individual risk factors, previous findings, symptoms, and personal preference rather than simply adding the test because it is available.

What Does Thyroid Ultrasound Examine?

A thyroid ultrasound evaluates:

  • Thyroid size and overall appearance
  • Thyroid nodules and cysts
  • Ultrasound characteristics of thyroid nodules
  • Structures surrounding the thyroid
  • Cervical lymph nodes when included in the examination

Thyroid ultrasound is commonly offered as an optional examination or included in higher-level private health screening packages in Korea.

What Is Thyroid Ultrasound Good At?

Thyroid ultrasound is very good at detecting and characterizing structural abnormalities.

Common findings include:

  • Thyroid nodules
  • Simple or predominantly cystic nodules
  • Multiple nodules
  • Thyroid enlargement
  • Changes associated with chronic thyroiditis
  • Suspicious thyroid nodules
  • Abnormal cervical lymph nodes

It can also be used to guide fine-needle aspiration when biopsy is needed.

However, ultrasound cannot determine thyroid hormone function.

A thyroid gland may appear normal on ultrasound while thyroid function is abnormal, and a person may have multiple thyroid nodules while TSH and free T4 remain normal.

For this reason, thyroid ultrasound and thyroid blood tests answer different questions.

Thyroid Nodules Are Very Common

Finding a thyroid nodule on ultrasound is common, particularly with increasing age.

Many nodules are only a few millimeters in size and would never have caused symptoms or been noticed without imaging.

The most important question is therefore not simply:

“Is there a nodule?”

It is:

“Does this nodule have ultrasound features that make it clinically important?”

Both size and ultrasound appearance matter.

What Features Are Evaluated?

When a thyroid nodule is found, the examiner assesses features such as:

  • Solid or cystic composition
  • Echogenicity
  • Shape
  • Margins
  • Echogenic foci
  • Relationship to the thyroid capsule
  • Suspicious cervical lymph nodes

Certain combinations of features are associated with a higher risk of thyroid cancer.

For example, punctate echogenic foci may be a suspicious feature in the appropriate context, while other types of echogenic foci or calcification may have different significance.

No single ultrasound feature by itself diagnoses cancer.

This is why standardized risk-classification systems are used.

What Is K-TIRADS?

In Korea, thyroid nodules are commonly classified using K-TIRADS, the Korean Thyroid Imaging Reporting and Data System.

K-TIRADS categorizes nodules according to their ultrasound pattern and estimated malignancy risk.

The categories are broadly:

  • K-TIRADS 1: no nodule
  • K-TIRADS 2: benign
  • K-TIRADS 3: low suspicion
  • K-TIRADS 4: intermediate suspicion
  • K-TIRADS 5: high suspicion

A higher K-TIRADS category means that the ultrasound appearance is more suspicious.

It does not mean that cancer has already been diagnosed.

K-TIRADS is used together with nodule size and other clinical findings to determine whether follow-up or biopsy is appropriate.

Does Every Nodule Need a Biopsy?

No.

One of the most important principles in thyroid ultrasound is that not every thyroid nodule should undergo fine-needle aspiration.

Biopsy decisions depend mainly on:

  • K-TIRADS category
  • Nodule size
  • Suspicious cervical lymph nodes
  • Possible invasion of surrounding structures
  • Clinical risk factors
  • Symptoms
  • Previous growth

In general, biopsy is considered at smaller sizes for more suspicious nodules and at larger sizes for lower-suspicion nodules.

Approximate K-TIRADS biopsy thresholds are:

  • K-TIRADS 5: generally over 1 cm
  • K-TIRADS 4: generally over 1–1.5 cm
  • K-TIRADS 3: generally over 2 cm
  • K-TIRADS 2: usually no biopsy

These are not absolute rules.

A smaller nodule may require biopsy when there are important high-risk findings, such as suspicious lymph nodes or invasion of adjacent structures.

Conversely, many small nodules can simply be monitored.

What Is Fine-Needle Aspiration?

Fine-needle aspiration, or FNA, uses a thin needle to collect cells from a thyroid nodule under ultrasound guidance.

It is usually performed as an outpatient procedure and generally takes only a few minutes.

The cytology result may be reported as:

  • Benign
  • Nondiagnostic
  • Indeterminate
  • Suspicious
  • Malignant

An indeterminate result does not automatically mean cancer.

Depending on the result and clinical situation, the next step may include:

  • Repeat FNA
  • Continued ultrasound follow-up
  • Additional testing in selected cases
  • Surgery

Does FNA Hurt?

Most people feel a brief needle prick and some pressure during the procedure.

Mild neck soreness, tenderness, or bruising may occur afterward, but significant pain is uncommon.

If discomfort occurs, it is usually manageable with an ice pack or a simple pain reliever.

How Long Is the Recovery?

Recovery is usually quick.

Most people can leave shortly after the procedure and return to normal daily activities the same day.

Mild tenderness may continue for a day or two.

Significant neck swelling, persistent bleeding, or severe or worsening pain is uncommon and should be medically evaluated if it occurs.

How Much Does FNA Cost in Korea?

The cost of thyroid FNA varies considerably.

Unlike a screening thyroid ultrasound, FNA is usually performed because a thyroid nodule has already been identified and meets clinical criteria for further evaluation.

The final cost may depend on:

  • Whether Korean National Health Insurance applies
  • Ultrasound-guidance fees
  • Cytology or pathology fees
  • The institution where the procedure is performed
  • Whether additional testing is required

Because these components may be billed differently, a single screening-style price range can be misleading.

If FNA is recommended, ask for the expected total cost including ultrasound guidance and cytology.

What About Very Small Suspicious Nodules?

Modern ultrasound can detect thyroid cancers smaller than 1 cm.

However, many small papillary thyroid cancers grow very slowly.

For selected adults with low-risk papillary thyroid microcarcinoma, active surveillance rather than immediate surgery may be considered.

This generally applies to carefully selected tumors measuring 1 cm or less without concerning features such as:

  • Suspicious lymph-node metastasis
  • Distant metastasis
  • Significant invasion of surrounding structures

This is another reason why finding a very small suspicious thyroid nodule does not automatically mean that immediate surgery is necessary.

Thyroid Cysts

Thyroid cysts are common.

A purely cystic nodule has a very low risk of malignancy and is generally considered benign on ultrasound.

Most asymptomatic cysts do not require treatment.

A large or recurrent cyst may occasionally cause:

  • Neck pressure
  • Difficulty swallowing
  • Visible swelling
  • Local discomfort

In those situations, aspiration or another treatment may be considered.

Multiple Thyroid Nodules

Many people have more than one thyroid nodule.

Reports may use terms such as:

  • Multiple thyroid nodules
  • Multinodular goiter
  • Multiple benign-appearing nodules

Having several nodules does not automatically mean a higher cancer risk.

Each clinically relevant nodule is evaluated according to its own ultrasound appearance.

The largest nodule is not necessarily the most important one.

A smaller nodule with suspicious features may require more attention than a larger benign-appearing nodule.

Thyroiditis on Ultrasound

Ultrasound may sometimes show findings suggestive of chronic thyroiditis, such as:

  • Heterogeneous thyroid echotexture
  • Decreased echogenicity
  • Diffuse structural change

Hashimoto thyroiditis is one possible cause.

However, ultrasound alone does not determine thyroid function.

If thyroiditis is suspected, blood tests such as:

  • TSH
  • Free T4
  • Thyroid antibodies when appropriate

may provide additional information.

A person may have ultrasound findings of chronic thyroiditis while thyroid hormone levels remain normal.

What About Cervical Lymph Nodes?

A thyroid ultrasound may also assess cervical lymph nodes, particularly when a suspicious thyroid nodule is present.

Suspicious lymph-node findings can significantly change management.

If a lymph node appears suspicious for thyroid cancer metastasis, biopsy may be recommended even when the thyroid nodule itself is relatively small.

This is one reason a detailed diagnostic thyroid and neck ultrasound may differ from a brief screening examination.

Who Should Consider Thyroid Ultrasound?

Routine thyroid ultrasound screening is not recommended for every asymptomatic adult.

However, it may be particularly appropriate when there is:

  • A palpable thyroid nodule
  • Visible or asymmetric thyroid enlargement
  • A thyroid abnormality found on another imaging test
  • Suspicious cervical lymph-node enlargement
  • A previously known thyroid nodule requiring follow-up
  • Previous thyroid cancer
  • A physician’s concern based on examination or symptoms
  • Certain high-risk clinical histories

People with a significant family history of thyroid cancer or previous childhood neck irradiation may also require individualized assessment.

The decision should be based on clinical context rather than simply adding ultrasound because it is available.

Why Is Routine Screening Controversial?

Thyroid ultrasound can detect very small cancers that might never have caused symptoms or affected a person’s lifespan.

This can lead to:

  • Repeated ultrasound examinations
  • Fine-needle aspiration
  • Anxiety
  • Surgery
  • Lifelong thyroid hormone replacement after some operations
  • Treatment of tumors that may never have become clinically important

This phenomenon is known as overdiagnosis.

For this reason, modern thyroid management increasingly emphasizes:

  • Avoiding unnecessary biopsy
  • Using risk-based ultrasound classification
  • Monitoring many small nodules rather than intervening immediately
  • Considering active surveillance for selected low-risk cancers

The goal should not be to detect every tiny thyroid nodule.

The goal is to identify abnormalities that are likely to matter clinically.

Do You Need to Fast?

No.

Thyroid ultrasound does not require fasting.

You can generally eat, drink, and take usual medications normally unless other examinations scheduled on the same day require fasting.

For example, a Korean health screening center may still instruct you to fast because abdominal ultrasound or gastroscopy is being performed during the same visit.

The fasting requirement is not for the thyroid ultrasound itself.

How Is the Examination Performed?

You usually lie on your back with your neck slightly extended.

Gel is applied to the front and sides of the neck, and a high-frequency ultrasound probe is moved over the thyroid.

The examination is:

  • Noninvasive
  • Radiation-free
  • Usually painless
  • Performed without contrast material

No needles are involved unless a separate biopsy procedure is scheduled.

How Long Does It Take?

A routine thyroid ultrasound is usually a short examination.

The time depends on factors such as:

  • Number of nodules
  • Presence of previous lesions requiring comparison
  • Need for detailed lymph-node evaluation
  • Complexity of the findings

A patient with multiple nodules or a suspicious lesion may require a more detailed examination than someone with a normal thyroid.

How Much Does Thyroid Ultrasound Cost in Korea?

Thyroid ultrasound is commonly offered as an optional examination in Korean health screening centers and may already be included in some comprehensive packages.

A practical estimate for a screening thyroid ultrasound is approximately:

₩80,000–120,000 (about US$53–80)

using an approximate exchange rate of ₩1,500 per US dollar.

Prices vary depending on the institution and whether the examination is performed as part of a health screening package or as a separate diagnostic examination.

Always check whether thyroid ultrasound is already included in your package before adding it separately.

What Happens If a Nodule Is Found?

The next step depends mainly on ultrasound appearance and size.

Possible outcomes include:

  • No follow-up
  • Repeat ultrasound after an interval
  • Comparison with previous imaging
  • Thyroid blood tests
  • Fine-needle aspiration
  • Specialist consultation

Finding a thyroid nodule does not automatically mean that a biopsy is needed.

Likewise, undergoing a biopsy does not mean that surgery will be required.

Risk classification and size thresholds are used partly to avoid unnecessary procedures for nodules that are unlikely to cause harm.

What to Ask When Booking

Before booking, ask:

  • Is this a thyroid ultrasound or a thyroid and neck ultrasound?
  • Are cervical lymph nodes evaluated?
  • Is thyroid ultrasound already included in my screening package?
  • Is an English report available?
  • Will the report include nodule size and K-TIRADS classification?
  • Can previous ultrasound images or reports be compared?

If you already have a known thyroid nodule, tell the center before the examination.

A follow-up study is most useful when the same lesion can be compared with previous measurements and images.

Tips for Foreigners

Bring previous thyroid ultrasound reports or images if available.

This is especially useful if you have previously been told that you have:

  • A thyroid nodule
  • A thyroid cyst
  • Multinodular goiter
  • Hashimoto thyroiditis
  • A previous thyroid biopsy
  • Previous thyroid surgery

Some overseas reports may use a different thyroid ultrasound classification system, such as ACR TI-RADS.

These categories are not directly interchangeable with K-TIRADS, so bring the previous report or images rather than comparing category numbers alone.

When possible, bring the actual ultrasound images as well as the written report.

Growth is easier to assess when the current examiner can compare the same lesion with previous imaging.

Final Thoughts

Thyroid ultrasound is excellent at detecting thyroid nodules.

That does not mean everyone needs to be screened or that every detected nodule needs treatment.

The most important questions are:

  • What does the nodule look like?
  • What is its K-TIRADS category?
  • How large is it?
  • Are there suspicious lymph nodes or invasive features?
  • Has it changed over time?

Most thyroid nodules are benign, and many small suspicious nodules can also be managed conservatively.

Routine thyroid ultrasound screening is not recommended for every asymptomatic adult, even though it is widely available in Korean private health checkups.

If FNA becomes necessary, it is usually a brief outpatient procedure with mild discomfort and little recovery time. The cost is more variable than a screening ultrasound because insurance coverage, ultrasound guidance, and cytology fees may differ.

The value of thyroid ultrasound comes from appropriate risk classification and avoiding unnecessary intervention, not simply from detecting the greatest possible number of small nodules.
