Brain CT in Korean Health Checkups: Is It Really Useful?

Brain CT, also called head CT, is sometimes included in comprehensive health checkup packages in Korea.

In some programs, it is bundled with other CT examinations as part of a premium screening package.

Because of this, brain CT can sound like a comprehensive way to “check the brain.” In reality, its value in routine health screening is more limited.

Brain CT can detect several important abnormalities, but for most conditions involving brain tissue, MRI provides substantially more information.

What Is a Brain CT?

Brain CT uses X-rays to create cross-sectional images of the head.

A routine brain CT performed as part of a health checkup is usually done without intravenous contrast.

It is quick, widely available, and generally takes only a few minutes.

What Can Brain CT Show — and What Are Its Limitations?

A routine non-contrast brain CT can detect findings such as:

  • intracranial calcification
  • larger old cerebral infarctions
  • cerebral atrophy
  • hydrocephalus
  • intracranial hemorrhage
  • some larger brain masses
  • skull abnormalities
  • sinus abnormalities within scan range

These findings can occasionally be clinically useful, especially when they were previously unknown.

However, CT is relatively insensitive to many subtle abnormalities involving brain tissue.

Small cerebral infarctions, white-matter abnormalities, many small tumors, posterior fossa lesions, and other subtle structural abnormalities may be difficult to detect or completely invisible on routine CT.

A normal brain CT therefore does not mean that the brain has been comprehensively evaluated.

Brain CT vs. Brain MRI

For elective brain imaging in an asymptomatic person, MRI is generally the more informative examination.

MRI is more sensitive for:

  • small cerebral infarctions
  • white-matter disease
  • brain tumors
  • posterior fossa abnormalities
  • demyelinating disease
  • subtle structural abnormalities

MRI also does not use ionizing radiation.

CT has a clear advantage for calcification, which is easier to detect and characterize on CT.

CT is also extremely useful in emergency medicine because it is fast and can rapidly identify intracranial hemorrhage or major traumatic abnormalities.

But those advantages are much less important in routine health screening.

If the goal is simply to obtain a detailed structural evaluation of the brain, MRI usually provides more useful information than routine non-contrast CT.

What About Cerebral Aneurysms?

A routine non-contrast brain CT does not adequately evaluate cerebral arteries and should not be considered an aneurysm screening test.

When screening for cerebral aneurysm is appropriate, brain MRA is commonly used as a non-invasive examination.

MRA can evaluate cerebral arteries without ionizing radiation and can often be performed without intravenous contrast.

If MRA shows a suspected aneurysm or an indeterminate vascular abnormality, CT angiography (CTA) may be performed for more detailed anatomical assessment.

CTA can help evaluate features such as:

  • aneurysm size
  • aneurysm shape
  • neck anatomy
  • calcification
  • relationship to nearby vessels

In selected cases, catheter angiography may still be needed for definitive evaluation or treatment planning.

The important point is that having a routine brain CT does not mean that cerebral arteries have also been screened.

Why Is Brain CT Included in Some Health Checkup Packages?

Brain CT has several practical advantages for screening centers.

It is:

  • quick
  • widely available
  • relatively inexpensive
  • easy to add to a package
  • usually performed without contrast

For this reason, it may be bundled with chest CT, abdominal CT, or other examinations in premium health checkup programs.

But ease of inclusion does not necessarily mean high screening value.

A package containing more CT examinations may look more comprehensive, but brain CT may add relatively little information in a healthy person without neurological symptoms or specific risk factors.

This is worth considering when comparing health checkup packages.

More imaging does not automatically mean better screening.

Who Might Still Benefit From Brain CT?

Brain CT may still be useful in selected situations, such as:

  • known previous cerebral infarction
  • previous intracranial abnormality requiring follow-up
  • abnormality identified on another examination
  • inability to undergo MRI
  • specific need to evaluate calcification or skull abnormality

If you have new neurological symptoms, however, this is no longer routine screening.

Sudden weakness, difficulty speaking, severe acute headache, loss of consciousness, or other new neurological symptoms require proper diagnostic evaluation rather than a health checkup.

How Much Radiation Is There?

Brain CT uses ionizing radiation.

Radiation exposure from a single head CT is lower than that from many body CT examinations, but it is not zero.

This matters more when CT is being considered as an optional examination in an otherwise healthy person, rather than because of a clear medical indication.

MRI has an advantage in this setting because it does not use ionizing radiation.

The radiation risk from a single brain CT is small, but unnecessary repeated CT examinations should still be avoided.

How Much Does a Brain CT Cost in Korea?

The cost varies depending on the hospital, screening center, and whether brain CT is purchased separately or included in a comprehensive package.

As a rough guide, a non-contrast brain CT may cost around ₩100,000–₩200,000, or approximately US$70–140, based on an exchange rate of about US$1 = ₩1,417 in August 2026.

If brain CT is already included in a comprehensive health checkup package, there may be little or no additional charge.

However, the fact that it is included does not necessarily mean that it adds substantial screening value.

If you are specifically choosing and paying for brain imaging, it may be more useful to compare the expected information from brain MRI rather than selecting CT simply because it is cheaper.

Do You Need to Fast?

For a routine non-contrast brain CT, fasting is generally not required.

There is usually:

  • no IV contrast
  • no special dietary preparation
  • no lengthy scanning process

However, if brain CT is performed as part of a comprehensive health checkup, you may still need to fast because of other examinations in the same package.

Follow the instructions provided for the entire health checkup.

The Bottom Line

Brain CT can detect real and sometimes important abnormalities, including calcification, larger old infarctions, hydrocephalus, hemorrhage, some larger masses, and certain structural abnormalities.

But its role in routine health screening is limited.

In Korean health checkup programs, brain CT may be included because it is quick, relatively inexpensive, and easy to perform. That does not mean it provides a comprehensive assessment of brain health.

For most abnormalities involving brain tissue, MRI provides substantially more information and does not involve radiation exposure.

Routine brain CT also does not adequately evaluate cerebral arteries. If aneurysm screening is appropriate, MRA is generally more relevant, with CTA sometimes used when more detailed evaluation is needed.

So if brain CT is already included in your package, it may still provide some useful information.

But if you are specifically choosing and paying for brain imaging, MRI is usually the more informative test.
