CT Scans in Korean Health Checkups: A Guide for Foreigners

For people who have never smoked and have no major risk factors, routine annual chest CT may provide less benefit and may lead to incidental findings and repeated follow-up scans.

What Happens If a Lung Nodule Is Found?

Lung nodules are common, and many are benign.

The recommended next step depends on factors such as:

  • Nodule size
  • Shape and appearance
  • Whether it is solid or ground-glass
  • Smoking history
  • Previous CT results
  • Whether the nodule changes over time

Some nodules require no immediate action, while others require follow-up CT or specialist consultation.

Bring previous chest CT images or reports whenever possible. Comparison with earlier scans is often important.

Coronary Artery Calcium CT

A coronary artery calcium CT measures calcium within the walls of the coronary arteries.

It is usually performed without contrast and produces a calcium score.

The test may help assess the likelihood of coronary atherosclerosis and refine cardiovascular risk in selected adults.

It does not directly show whether every coronary artery is narrowed, and a calcium score of zero does not exclude all forms of coronary artery disease.

This examination is different from coronary CT angiography.

Who May Benefit?

Calcium scoring may be useful when a person’s cardiovascular risk is uncertain and the result could influence decisions about preventive treatment.

Its value depends on factors including:

Coronary CT Angiography

Coronary CT angiography, also called coronary CTA or CCTA, is a more complex cardiac CT examination.

Unlike coronary calcium scoring, it uses intravenous contrast to show the coronary artery lumen and plaque. It can help evaluate whether plaque has caused significant narrowing of the coronary arteries.

Brain CT

Non-contrast brain CT may be offered as an optional examination in some Korean health screening packages.

It can show:

  • Some old strokes or calcifications

However, brain CT is not the best test for every neurological condition.

MRI generally provides better soft-tissue detail for many brain abnormalities, including small tumors, subtle infarctions, white matter disease, and some vascular or inflammatory conditions.

Routine brain CT in a healthy person without symptoms or specific risk factors has limited screening value. It also exposes the head to radiation and may detect incidental findings that require additional evaluation.

People with sudden weakness, speech difficulty, severe acute headache, loss of consciousness, or other new neurological symptoms need prompt medical assessment rather than a routine health checkup CT.

Cervical and Lumbar Spine CT

Cervical or lumbar spine CT may be offered as an optional test for the neck or lower back.

CT is particularly good at showing:

  • Bone structure
  • Fractures
  • Bone spurs
  • Spinal alignment
  • Some degenerative changes

However, MRI is usually better for evaluating:

  • Intervertebral discs
  • Nerves
  • Spinal cord
  • Soft-tissue abnormalities

Degenerative changes are common even in people without symptoms. Therefore, finding a disc bulge or bone spur on screening CT does not necessarily mean it is causing pain.

Spine imaging is generally more useful when selected according to symptoms, examination findings, previous injury, or a clinician’s recommendation.

Abdominal and Pelvic CT

Abdominal and pelvic CT provides detailed images of organs and structures such as the:

  • Liver

  • Kidneys

  • Pelvic organs

Unlike routine abdominal ultrasound, abdominal and pelvic CT can evaluate deeper structures and areas obscured by bowel gas.

However, the value of the examination depends heavily on the scanning protocol.

Is Contrast Needed?

Contrast-enhanced CT is usually more informative than a simple non-contrast scan when evaluating many abdominal organs, blood vessels, inflammatory conditions, and masses.

A non-contrast abdominal CT may still be useful for certain purposes, such as evaluating urinary stones, calcification, or visceral fat. However, it should not be assumed to provide the same information as a contrast-enhanced abdominal CT.

Some liver, kidney, pancreatic, uterine, or ovarian abnormalities may be better evaluated with MRI or another dedicated imaging protocol.

Visceral Fat CT

  • Whether multiple phases are acquired

Published reference values for typical adult CT examinations include:

  • Low-dose lung cancer screening CT: about 1.5 mSv
  • Brain CT: about 1.6 mSv
  • Coronary artery calcium CT: about 1.7 mSv
  • Standard chest CT: about 6.1 mSv
  • Abdominal and pelvic CT: about 7.7 mSv

These are general reference values rather than exact doses for every patient or scanner. Actual radiation exposure may be lower or higher depending on the equipment, patient size, scan range, and imaging protocol.

For comparison, average natural background radiation is approximately 3 mSv per year, although this varies by location.

A single medically appropriate CT examination generally carries a small radiation risk. In health screening, the greater concern is cumulative exposure from repeated or unnecessary scans over time.

Keep a record of previous CT examinations and avoid repeating the same scan simply because it is included in a health checkup package.

How to Prepare for a CT Scan

Preparation depends mainly on whether contrast is used.

CT Without Contrast

For low-dose chest CT, coronary calcium CT, and many brain or spine CT examinations:

  • Fasting is usually not required.
  • Regular medication can usually be taken.
  • Metal objects may need to be removed.
  • You may be asked to change into a gown.

Follow the center’s specific instructions.

Contrast-Enhanced CT

For a CT using intravenous contrast, you may be instructed to:

  • Fast for several hours

Which CT Should You Choose?

Do not choose CT only because it is included in a premium package.

Low-Dose Chest CT

Coronary Calcium CT

Brain CT

Abdominal and Pelvic CT

May be appropriate for:

  • Further evaluation of an abnormal ultrasound or blood test

  • Specific symptoms or clinical risk
  • A targeted physician recommendation

Routine repeated abdominal CT in an asymptomatic, low-risk person should be weighed against radiation exposure and the possibility of incidental findings.

Incidental Findings and Follow-Up

CT may detect abnormalities unrelated to the original reason for the examination. These are called incidental findings.

Examples include:

  • Lung nodules
  • Liver, pancreatic, kidney, or adrenal lesions
  • Thyroid nodules
  • Coronary artery calcification
  • Small brain abnormalities
  • Enlarged lymph nodes

Incidental findings can occasionally lead to early detection of an important disease. However, they can also result in anxiety, repeated imaging, additional costs, biopsy, and further radiation exposure even when the final diagnosis is benign.

Before undergoing optional CT screening, consider whether you will be able to receive the full report and arrange appropriate follow-up if an abnormality is found.

What to Ask When Booking

Before adding a CT examination, ask:

  • What exact body area will be scanned?
  • Is this a low-dose or standard-dose protocol?
  • Is intravenous contrast used?
  • Is fasting required?
  • Do I need a kidney function test?
  • Does the scan include both the abdomen and pelvis?
  • Is this a full diagnostic CT or a limited screening scan?
  • Who interprets the images?
  • Can I receive the report in English?
  • Can I receive the original images?
  • What happens if an abnormality is found?
  • Are contrast and follow-up fees included?

The test name alone may not describe the full protocol. For example, “heart CT” may mean either non-contrast coronary calcium scoring or contrast-enhanced coronary CT angiography.

Tips for Foreigners

Ask the center to provide the exact English name of the CT examination before booking.

Bring previous CT reports and images, especially if you have a known lung nodule, liver lesion, kidney lesion, adrenal nodule, coronary calcium score, or spinal abnormality.

Tell the center about previous contrast reactions and kidney disease in advance rather than waiting until the day of the examination.

Confirm whether the written report, images, and follow-up consultation are available in English.

Do not assume that the most expensive package is the most appropriate. A smaller number of carefully selected tests is often more useful than several unrelated scans.

Final Thoughts

CT can provide detailed and clinically useful images, but different CT examinations have very different purposes.

Low-dose chest CT is the most established screening CT for appropriately selected people at high risk of lung cancer. Coronary calcium CT may help refine cardiovascular risk in selected adults.

Brain and spine CT generally have limited value as routine screening tests for healthy people without relevant symptoms or risk factors.

Abdominal and pelvic CT can provide detailed information, but the protocol and use of contrast strongly affect what the examination can detect.

Before choosing a CT scan, confirm:

  • The exact scanning area
  • Whether contrast is used
  • The radiation dose or low-dose protocol
  • Preparation requirements
  • The reason the test is being recommended
  • The follow-up plan for abnormal findings
