MRI Scans in Korean Health Checkups: A Guide for Foreigners

MRI is offered as an optional test in many private health checkup programs in Korea.

MRI does not use ionizing radiation, but it takes longer than CT, is sensitive to movement, and requires careful screening for metal implants and medical devices.

What Is an MRI Scan?

Magnetic resonance imaging, usually called MRI, uses a strong magnetic field, radio waves, and computer processing to create detailed images of the body.

MRI is particularly useful for examining:

  • Brain
  • Spinal cord
  • Intervertebral discs and nerves
  • Muscles and joints
  • Pancreas and bile ducts
  • Female pelvic organs
  • Other soft tissues

Unlike CT, MRI does not use X-rays or ionizing radiation.

During the examination, you lie on a table that moves into a cylindrical scanner. The machine produces loud knocking, tapping, or buzzing sounds while images are being acquired.

You must remain still because movement can reduce image quality.

Most health screening MRI examinations take approximately 20–60 minutes. Combined or specialized examinations may take longer.

MRI, MRA, and MRCP: What Is the Difference?

The names can be confusing when selecting optional tests.

MRI

MRI generally refers to imaging of organs, soft tissues, bones, joints, or the nervous system.

For example:

  • Brain MRI evaluates brain tissue.
  • Spine MRI evaluates the spinal cord, discs, nerves, and surrounding structures.
  • Pelvic MRI evaluates female pelvic organs and other pelvic soft tissues.
  • Pancreatic or abdominal MRI evaluates selected abdominal organs.

MRA

Magnetic resonance angiography, or MRA, is an MRI technique designed to examine blood vessels.

Brain MRA is commonly offered in Korean checkup programs to evaluate intracranial arteries.

Depending on the protocol, MRA may be performed without contrast or with intravenous contrast.

Brain MRI and brain MRA are not the same examination.

Brain MRI primarily evaluates brain tissue, while brain MRA primarily evaluates blood vessels.

MRCP

Magnetic resonance cholangiopancreatography, or MRCP, is an MRI technique used to examine the:

  • Pancreatic duct
  • Bile ducts
  • Gallbladder
  • Surrounding pancreatic and biliary structures

MRCP is usually noninvasive and can often be performed without intravenous contrast.

Some Korean centers use terms such as:

  • Pancreatic MRI
  • Pancreas MRI
  • MRCP
  • Abdominal MRI
  • Pancreatic MRI with MRCP

These names should not be assumed to describe the same examination. Confirm the exact scanning range and protocol before booking.

MRI With or Without Contrast

MRI may be performed with or without intravenous contrast.

Non-Contrast MRI

Many health screening MRI examinations are performed without contrast, including:

  • Routine brain MRI
  • Brain MRA
  • Cervical spine MRI
  • Lumbar spine MRI
  • Some female pelvic MRI examinations
  • Some pancreatic MRI or MRCP examinations

Non-contrast MRI avoids an injection and usually requires less preparation.

Brain MRA

Brain MRA examines the intracranial arteries.

It may identify:

  • Cerebral aneurysms
  • Narrowing of intracranial arteries
  • Certain vascular malformations
  • Other blood vessel abnormalities

Brain MRA is commonly offered either alone or together with brain MRI in Korean health checkup programs.

A non-contrast time-of-flight MRA is frequently used because it can visualize many intracranial arteries without an injection.

However, MRA has limitations.

Small vessels, slow blood flow, movement, and technical artifacts can make a blood vessel appear narrowed or abnormal.

An abnormal screening MRA may require comparison with previous imaging or further evaluation with dedicated vascular imaging or specialist consultation.

Brain MRI and MRA Combined

Some centers offer brain MRI and brain MRA as a combined package.

This provides information about both:

  • Brain tissue
  • Major intracranial blood vessels

A combined examination is more comprehensive than either test alone, but it takes longer and costs more.

It may be reasonable when both structural brain disease and cerebrovascular abnormalities are relevant concerns.

However, choosing both examinations simply because they are sold as a package does not guarantee meaningful benefit for every healthy person.

Cervical Spine MRI

Cervical spine MRI examines the neck portion of the spine.

It provides detailed images of the:

  • Intervertebral discs
  • Spinal cord
  • Nerve roots
  • Spinal canal
  • Ligaments
  • Bone marrow
  • Surrounding soft tissues

It may identify:

  • Disc herniation
  • Spinal stenosis
  • Spinal cord compression
  • Nerve root compression
  • Tumors
  • Infection

  • Previous injury

MRI is usually more informative than CT when the main concern involves discs, nerves, or the spinal cord.

Lumbar Spine MRI

Lumbar spine MRI examines the lower back.

Seek urgent medical evaluation rather than routine screening if back pain is accompanied by:

  • New leg weakness
  • Loss of bladder or bowel control
  • Numbness around the groin
  • Fever
  • Major trauma

Pancreatic MRI and MRCP

Some Korean hospitals and higher-level health screening centers offer pancreatic MRI or MRCP as optional examinations.

MRI and MRCP are particularly useful for detecting and characterizing pancreatic cystic lesions.

They may show small cysts that are difficult to identify on ultrasound or routine CT. MRCP can also help demonstrate whether a cyst communicates with the pancreatic duct.

These examinations may evaluate:

  • Pancreatic cystic lesions
  • Pancreatic duct dilatation
  • Bile duct abnormalities
  • Gallstones within the bile duct
  • Other structural abnormalities of the pancreas and biliary system

The exact protocol varies between centers.

Some examinations are abbreviated non-contrast screening studies, while others are more detailed pancreatic MRI examinations that include contrast.

  • Whether MRCP is included
  • Whether intravenous contrast is used
  • Whether the scan is a dedicated pancreatic protocol
  • Whether the examination is intended for screening or evaluation of a known abnormality

Abdominal MRI

Some centers offer an examination labeled simply as “abdominal MRI.”

This may include selected upper abdominal organs such as the:

  • Pancreas
  • Liver
  • Gallbladder and bile ducts
  • Spleen
  • Kidneys
  • Adrenal glands

However, the name “abdominal MRI” does not guarantee that every abdominal organ is evaluated in equal detail.

In some centers, abdominal MRI is primarily designed to evaluate the pancreas and biliary system. In others, it may be a broader upper abdominal examination.

The protocol may also differ in whether it includes:

  • MRCP
  • Diffusion-weighted imaging
  • Intravenous contrast
  • Multiple contrast phases
  • A dedicated organ-specific sequence

Confirm the exact organs included before booking.

Female Pelvic MRI

Some Korean health screening centers offer pelvic MRI as an optional examination for women.

It may evaluate the:

  • Uterus
  • Endometrium
  • Ovaries

Pelvic ultrasound is more commonly used as the initial imaging examination in health checkups.

  • Pelvic organs were not adequately visualized on ultrasound
  • Ultrasound detected an indeterminate abnormality
  • There is a known gynecologic condition

Before booking, confirm:

  • Whether the examination is specifically designed for female pelvic organs
  • Whether intravenous contrast is used

MRI Scanner Strength: 1.5T vs. 3T

MRI scanner strength is measured in tesla, abbreviated as T.

Most modern Korean hospitals and health screening centers use either:

  • 1.5T MRI
  • 3T MRI

For an elective health screening MRI, a modern 3T scanner is generally preferable when available, particularly for brain MRI, brain MRA, spine MRI, and examinations that require detailed evaluation of small structures.

A 3T scanner provides a higher signal than a 1.5T scanner. This additional signal can be used to obtain:

  • Thinner image slices
  • Higher spatial resolution
  • Better visualization of small lesions
  • More detailed vascular images
  • Shorter scanning times for some sequences

The advantage may be particularly relevant when examining small brain lesions, intracranial blood vessels, spinal nerves, or small pancreatic cystic lesions.

However, field strength is not the only factor affecting image quality. A well-designed examination on a modern 1.5T scanner can still provide diagnostic-quality images.

Image quality also depends on:

  • Imaging protocol and sequences
  • Coil quality
  • Scanner generation
  • Patient movement
  • Scan duration
  • The technologist’s experience
  • The radiologist’s interpretation

In addition, 3T MRI may produce more susceptibility, chemical-shift, dielectric, or metal-related artifacts in certain situations. A 1.5T scanner may therefore be preferred for some patients with implanted hardware or when artifact is expected to interfere with the examination.

When both options are available for a routine elective screening examination, choosing a well-maintained 3T scanner with an appropriate protocol is generally reasonable. However, the exact protocol and interpretation remain more important than field strength alone.

How to Prepare for an MRI

Preparation depends on the body part and whether contrast or sedation is used.

Brain and Spine MRI

For most non-contrast brain or spine MRI examinations:

  • Fasting is usually not required.
  • Regular medication can usually be taken.
  • Metal objects must be removed.
  • You may be asked to change into a gown.

Pancreatic MRI, MRCP, and Abdominal MRI

Fasting is commonly required for several hours before pancreatic MRI, MRCP, or abdominal MRI.

Fasting may improve visualization of the gallbladder, bile ducts, and pancreas and reduce interference from stomach and bowel contents.

Follow the center’s instructions regarding food, water, and medication.

Female Pelvic MRI

Preparation varies according to the center and imaging protocol.

You may be asked to:

  • Fast for several hours
  • Empty your bladder about one hour before the examination and then drink water
  • Avoid urinating for approximately 30–60 minutes before the scan
  • Maintain a comfortably or moderately filled bladder rather than an extremely full bladder
  • Receive medication to reduce bowel movement

Do not deliberately arrive with either a completely empty or painfully full bladder unless the center specifically instructs you to do so. Follow the preparation instructions provided by the imaging center.

Contrast-Enhanced MRI

You may need:

  • An intravenous line
  • Kidney function testing
  • A review of previous contrast reactions
  • Additional fasting, depending on the examination

Sedation

Sedation is not routinely used for most adult health screening MRI examinations.

However, it may be considered for severe claustrophobia or inability to remain still.

Sedation requires additional monitoring, preparation, and recovery time. You may need someone to accompany you home.

MRI Safety and Metal

The strong MRI magnetic field can interact with certain metals, implants, and medical devices.

Tell the center before the appointment if you have:

  • A pacemaker or implantable defibrillator
  • A cochlear implant
  • An aneurysm clip
  • A neurostimulator
  • An insulin pump
  • An implanted medication pump
  • Metal fragments in the eye or body

Many modern implants are MRI conditional rather than completely prohibited.

Claustrophobia and Noise

MRI scanners are narrow and can feel enclosed.

Tell the center before booking if you have claustrophobia, panic disorder, or difficulty remaining still.

Possible options include:

  • Practicing slow breathing
  • Using earplugs or headphones
  • Keeping your eyes closed before entering the scanner
  • Using a mirror when available
  • Scheduling a wide-bore MRI

  • Sedation in selected cases

MRI machines produce loud sounds, so hearing protection should be provided.

Open MRI may feel more comfortable, but scanner strength and image quality vary. It may not be appropriate for every examination.

Pregnancy and MRI

MRI does not use ionizing radiation.

However, tell the center if you are pregnant or may be pregnant.

MRI may be performed during pregnancy when medically appropriate, but gadolinium contrast is generally avoided unless the expected benefit clearly outweighs the potential risk.

What to Ask When Booking

Before adding an MRI examination, ask:

  • What exact body part will be examined?
  • Is the test MRI, MRA, MRCP, or a combination?
  • Which organs are included?
  • Is the examination a full diagnostic protocol or a shortened screening protocol?
  • Is contrast used?
  • Is fasting required?
  • Do I need a kidney function test?

The test name alone may not describe the full protocol.

For example, a “brain MRI package” may include MRI alone, MRA alone, or both.

A “pancreatic MRI” may or may not include MRCP.

An “abdominal MRI” may focus mainly on the pancreas and bile ducts rather than every abdominal organ.

A “pelvic MRI” offered in a health screening program usually refers to female pelvic imaging, but the exact organs and sequences should still be confirmed.

Tips for Foreigners

Ask the center to provide the exact English name of the MRI examination. Similar Korean test names may refer to different scanning areas or protocols.

If you have an implanted medical device or previous surgery involving metal, send the device information to the center before your appointment. Bring the implant card, operative record, or manufacturer and model information whenever available, especially if the procedure was performed outside Korea.

Bring previous MRI, CT, and ultrasound reports and images. Comparison with earlier examinations may prevent unnecessary repeat imaging or follow-up.

Confirm whether the center can provide:

  • Preparation instructions in English
  • An English radiology report
  • The original MRI images
  • An English-speaking consultation if an abnormality is found

Do not rely only on an English package name shown on a booking page. Ask which body part, organs, and sequences are actually included.

Final Thoughts

MRI provides detailed images without ionizing radiation, but each MRI examination is designed for a different purpose.
