Brain MRI is commonly offered as an optional examination in comprehensive health checkup programs in Korea.
Compared with brain CT, MRI provides much more detailed information about brain tissue and does not use ionizing radiation.
It can detect small cerebral infarctions, white-matter changes, brain tumors, microbleeds, and many other abnormalities that may be difficult or impossible to see on routine CT.
For someone who specifically wants detailed imaging of the brain, MRI is generally the most informative structural imaging examination available.
However, that does not mean that every healthy person necessarily needs routine brain MRI.
What Is a Brain MRI?
MRI uses a strong magnetic field and radiofrequency signals to produce detailed images of the brain.
Unlike CT, it does not use X-rays or ionizing radiation.
A typical brain MRI consists of several different imaging sequences. Each sequence highlights different characteristics of brain tissue.
A health screening brain MRI commonly includes sequences that can evaluate:
- brain anatomy
- cerebral infarction
- white-matter abnormalities
- previous hemorrhage or microbleeds
- brain tumors
- hydrocephalus
- cerebral atrophy
- other structural abnormalities
The exact protocol varies between hospitals and screening centers.
Why Are There So Many Different MRI Sequences?
MRI does not produce just one type of image.
Different sequences provide different information.
For example:
- T1-weighted imaging shows detailed anatomy
- T2-weighted imaging highlights many abnormalities with increased water content
- FLAIR imaging is particularly useful for white-matter abnormalities and old ischemic changes

What Can Brain MRI Find?
Small or old cerebral infarctions
MRI is very sensitive for detecting cerebral infarctions.
DWI can identify relatively recent ischemic injury, while other sequences can show older infarctions that may have occurred without obvious symptoms.
This means MRI may occasionally identify a previous silent cerebral infarction during a health checkup.
However, finding no previous infarction does not mean that future stroke risk is low.
Stroke prevention still depends primarily on managing risk factors such as hypertension, diabetes, smoking, dyslipidemia, and atrial fibrillation.
White-matter changes
Small areas of abnormal signal in cerebral white matter are commonly seen on MRI, particularly with increasing age.
These changes may be associated with:
- aging
- hypertension
- diabetes
- other vascular risk factors
Mild white-matter changes are common and do not necessarily indicate a serious neurological disease.
The significance depends on age, extent, pattern, and clinical context.
Cerebral microbleeds
Sequences such as SWI can detect small deposits related to previous microscopic hemorrhage.
These cerebral microbleeds are generally not visible on routine brain CT.
Their significance varies depending on number, location, age, vascular risk factors, and other MRI findings.
Brain tumors
MRI is the preferred imaging method for detecting and characterizing most brain tumors.
Even without contrast, MRI can identify many structural brain lesions.
However, if a suspicious lesion is discovered, a contrast-enhanced MRI may be recommended for more detailed characterization.
Other structural abnormalities
Brain MRI can also identify abnormalities such as:
- hydrocephalus
- cerebral atrophy
- congenital structural abnormalities
- some inflammatory or demyelinating lesions
- previous traumatic brain injury
- abnormalities involving posterior fossa structures
The clinical importance of an incidental finding varies considerably.
Is Contrast Usually Needed for a Screening Brain MRI?
Usually not.
A routine brain MRI performed as part of a health checkup is commonly done without intravenous contrast.
A well-designed non-contrast MRI can already provide extensive information about brain structure, ischemic disease, white-matter abnormalities, previous hemorrhage, and many other conditions.
Gadolinium-based contrast may be added when there is a specific reason, such as:
- suspected brain tumor
- inflammatory or infectious disease
- certain cranial nerve abnormalities
- abnormality requiring further characterization
In other words, contrast is generally used to answer a specific diagnostic question, rather than being routinely necessary for an asymptomatic screening examination.
Brain MRI vs. Brain CT
For routine evaluation of brain tissue, MRI is substantially more informative than CT.
MRI is better for detecting:
- small cerebral infarctions
- white-matter disease
- most brain tumors
- cerebral microbleeds
- posterior fossa abnormalities
- subtle structural lesions
It also does not involve radiation exposure.
CT remains particularly good for calcification and is extremely useful in emergency situations because it is fast and can rapidly evaluate acute hemorrhage and trauma.
However, these CT advantages are less relevant in an elective health screening setting.
If you are specifically choosing between routine brain CT and brain MRI to obtain a detailed evaluation of the brain, MRI is generally the better examination.
Does Brain MRI Check the Brain Arteries?
Not necessarily.
A routine brain MRI primarily evaluates brain tissue.
Evaluation of cerebral arteries requires MR angiography (MRA), which is a different imaging technique and may be listed separately in a health checkup package.
Some Korean screening programs offer:
- brain MRI alone
- brain MRA alone
- brain MRI + MRA
If your main concern is cerebral aneurysm or intracranial arterial stenosis, check whether MRA is actually included.
A normal brain MRI should not automatically be interpreted as a normal cerebral angiographic examination.
Who Might Consider Brain MRI?
Brain MRI may be more useful for people with:
- significant cerebrovascular risk factors
- previous stroke or transient neurological symptoms
- known neurological disease requiring follow-up
- significant family or personal neurological history
- previous abnormal brain imaging
- a specific reason to evaluate brain structure in more detail
If new neurological symptoms are present, however, the situation should be evaluated clinically rather than simply handled through a routine health screening package.
For healthy, asymptomatic adults at average risk, there is no established recommendation that everyone should periodically undergo brain MRI.
What Are the Downsides of Screening Brain MRI?
MRI does not expose you to radiation, but that does not mean screening has no downside.
One important issue is incidental findings.
MRI is very sensitive and may identify abnormalities that are unrelated to symptoms and may never cause health problems.
These findings can sometimes lead to:
- repeat MRI
- contrast-enhanced MRI
- additional imaging
- specialist consultation
- anxiety about findings of uncertain significance
Therefore, detecting more abnormalities does not necessarily mean that screening improves health outcomes.
Does MRI Scanner Quality Matter?
Yes, although scanner specifications should not be considered in isolation.
Brain MRI in Korea is commonly performed on 1.5-T or 3-T systems.
Both can produce diagnostic brain MRI examinations when appropriate protocols are used.
A 3-T scanner generally provides higher signal-to-noise ratio and can offer advantages for thin-section imaging and certain advanced sequences, although image quality also depends heavily on:
- MRI protocol
- coil technology
- sequence selection
- acquisition time
- motion
- technologist experience
- radiologist interpretation
Simply seeing “3T MRI” in an advertisement does not guarantee that the examination is better.
A well-designed brain MRI protocol and experienced interpretation remain essential.
Is MRI Safe?
MRI does not use ionizing radiation and is considered very safe for most people.
However, the strong magnetic field means that MRI safety screening is important.
Tell the screening center if you have:
- pacemaker or other cardiac device
- cochlear implant
- neurostimulator
- vascular clip or other implanted device
- metallic foreign body
- previous surgery involving implanted hardware
Many modern implants are MRI-conditional and can be scanned safely under specified conditions, but this needs to be confirmed before the examination.
The ACR maintains detailed MRI safety recommendations because implant compatibility and scanning conditions vary by device.
What About Claustrophobia?
Brain MRI usually requires lying inside the scanner for considerably longer than CT.
Some people experience discomfort or claustrophobia.
If you have significant claustrophobia, tell the screening center before your appointment.
Options may include additional preparation, communication during scanning, or medication in selected cases.
Movement during MRI can degrade image quality, so remaining still is important.
How Much Does Brain MRI Cost in Korea?
The cost varies depending on the hospital, screening center, MRI scanner, and whether the examination is purchased separately or included in a comprehensive package.
As a rough guide, a non-contrast brain MRI may cost around ₩300,000–₩500,000, or approximately US$210–350, based on an exchange rate of about US$1 = ₩1,417 in August 2026.
The effective cost may be lower when brain MRI is included in a comprehensive health screening package.
Brain MRI combined with MRA may cost more than MRI alone.
If you are comparing packages, check whether the listed examination includes:
- brain MRI
- brain MRA
- or both
These are not the same examination.
Do You Need to Fast?
For a routine non-contrast brain MRI, fasting is generally not required.
However, if MRI is performed as part of a comprehensive Korean health checkup, fasting may still be required because of other examinations in the same package.
Follow the instructions provided by the screening center for the entire health checkup.
